Well, most likely a withdrawal from reality, minor delusions and hallucinations, and eventually, varying degrees by other emotional, behavioral, or intellectual disturbances. A very serious disorder, indeed. But with medication and professional help, he could lead a fairly normal, if invisible, life.

Unless, of course, you meant an invisible friend with split-personality disorder; now that would be hilarious.
